Client & Family Support Program
Esperanza’s Client and Family Support Program provides families who care for an individual with an Intellectual or Developmental disability with in-home case management and assistance with accessing benefits, supports, and resources. Esperanza is working to meet the unique needs of families and connect them with the resources they need in order to continue to keep their family members living in their home environment.
Building Confidence
Through working with our Client and Family staff, families build the knowledge, experience, and confidence they need to obtain these services independently. Through our support, Esperanza is helping families provide care to their loved ones in the most supportive environment there is—the family home.
Diverse Resources
Our program’s resources can include legal, medical and educational support, translation services, advocacy and more.
