Enrollment

Enrollment in Esperanza’s Client and Family Support Program begins with a phone call.  Esperanza staff will help answer your questions about the process of obtaining  Home Base Support Services (HBSS) funding and whether Esperanza is right for your family.

Funded individuals are accepted according to urgency of need and program capacity. If you receive HBSS funding, or are the guardian of an individual who does, please contact Laura Del Real,  Director of Client and Family Services, at 312.432.3944, x 17.
